Housing Stabilization Services (HSS) is a new service for individuals on Medical Assistance to help find and maintain housing.
Housing Stabilization Services has three parts:
Housing Consultation helps eligible individuals without a case manager or targeted case manager create a person-centered plan and help submit the plan to a Housing Transition provider.
Housing Transition helps eligible individuals find a home of their own.
Housing Sustaining to help maintain housing and prevent homelessness.
Who is Eligible?
A member is eligible for housing stabilization services if they meet all of the following needs-based criteria:
• Be on Medical Assistance (MA)
• Be 18 years old or older
• Proof of disability or disabling condition.
• Proof of housing instability.
• Proof of assessed need for services.
How does the program work?
Housing Stabilization Services provides up to 150 hours per transitional for housing transition and up to 150 hours for housing sustaining per year.
What does a Housing Stabilization Coordinator do?
A Housing Stabilization Coordinator works with the individual to find them the appropriate housing choice, while coordinating all the other necessary activities required for a successful transition into the community.
We work to remove any housing barriers such as finding funding sources, housing record expungement, getting a replacement social security card, etc.
